When rotary NiTi files are used in canals with flat-oval or tear-shaped cross sections, a circular bore is created, while the buccal and/or lingual recesses remain un-instrumented. It takes (a) the awareness that a given canal is flat and (b) expertise in creative use of hand instruments to try to overcome this problem. Tissue or biofilm remnants along such un-instrumented recesses may lead to failure due both to inadequate disinfection and to the inability to properly obturate the root-canal space

More Americans seeking dental treatment at the ER – Morris County Daily Record

People seeking treatment at Jersey Shore University Medical Center’s emergency room for dental care rose about 9 percent between 2010 and 2011.

That’s hardly just a local trend.

More Americans are heading to emergency rooms for dental problems like toothaches and other preventable conditions that could have been treated in a dentist’s office for a fraction of the cost, according to an analysis of government data and dental research.

The Pew Center on the States found that preventable dental conditions were the primary diagnosis in about 830,590 visits to ERs nationwide in 2009 — a 16 percent increase from 2006. For many low-income children, emergency rooms are the first and last resort because their families struggle to find a dentist who accepts Medicaid patients.

Sally Jo Placa, chairwoman of the Department of Dentistry at Jersey Shore University Medical Center in Neptune and a dentist in private practice, said the problem has grown as the economy has worsened.

“Employers are dropping dental insurance and insurance all together, so dental care and eye care – these kind of things become a luxury for a lot of people,” she said. “People are losing benefits and they’re losing jobs.”

The number of patients treated for dental problems at Jersey Shore jumped from 1,313 in 2010 to 1,430 in 2011.

It is not efficient care.

A toothache usually costs about $ 100 to treat by a dentist, she said.

“It’s upwards of $ 800 once they cross the threshold of the emergency room,” Placa said.

CT scans are the norm in emergency rooms, not simple X-rays, a much cheaper procedure performed in a dentist’s office.

“Emergency rooms are really the canary in the coal mine. If people are showing up in the ER for dental care, then we’ve got big holes in the delivery of care,” said Shelly Gehshan, director of Pew’s children’s dental campaign. “It’s just like pouring money down a hole. It’s the wrong service, in the wrong setting, at the wrong time.”

The typical emergency room treatment for a dental disorder is an antibiotic and pain reliever.

“That will get you better,” said Dr. Rajiv Arya, the associate director of the emergency department at Robert Wood Johnson University Hospital in New Brunswick. “But the underlying cause hasn’t been treated.”

Arya said there were at least 1,400 patients in 2011 at Robert Wood Johnson with a discharge diagnosis related to dental disease or toothaches.

Most of those emergency visits involve trouble such as toothaches that could have been avoided with regular checkups but went untreated, in many cases because of a shortage of dentists, particularly those willing to treat Medicaid patients, the analysis said.

Many ER dental visits involve the same patients seeking additional care since they are unable to find or afford follow-up treatment and end up back in the emergency room at a cost generally borne by the hospital.

“It’s not only a hardship for hospitals and taxpayers, but it is also taking nurses and emergency room physicians away from other issues,” Placa said.

A dental clinic at the Jersey Shore steers many patients away from the emergency room, but cannot serve everyone needing help.

“We have a waiting list and it’s grown exponentially in the last two years,” Placa said.

Marge Horning, 44, of Wall never sought treatment in an emergerncy room, always able to avoid it with things like a timely tax refund.

But she wasn’t always so lucky, sometimes putting off dental care because of the cost.

“I didn’t go for a while,” said Horning, who once had dental insurance but lost it years ago.

She chipped the bonding in a front tooth a month before being first treated for free at Jersey Shore’s dental clinic at the Jane H. Booker Family Health Center in Neptune in October 2010.

Horning was on the waiting list at the clinic for three years.

Pew researchers analyzed hospital information from 24 states, data from the federal Agency for Healthcare Research and Quality and studies on dental care. The center in Washington, D.C., is a division of the nonprofit Pew Charitable Trusts.

One recommendation the Pew report listed was to increase Medicaid payments to dentists so they pick up more low-income patients, an idea that Placa strongly endorses.

“Medicaid pays 28 to 32 cents on the dollar,” she said. “In New Jersey, a dentist’s overhead runs 70 percent. Seventy of every 100 cents goes to overhead expenses.”

Other potential steps include increasing water fluoridation and use of dental sealants.

Putting plastic sealants on molars can prevent cavities, but “children at the lowest risk are most likely to get them. It needs to be the opposite,” Gehshan said.

A shortage of dentists is not the problem in New Jersey, as it is elsewhere, especially rural areas, Placa said. There are 8,269 licensed dentists in New Jersey.
